Questions about Kentucky
Where do you order a certified Kentucky marriage certificate?
From the Office of Vital Statistics, part of the Cabinet for Health and Family Services in Frankfort. It has held marriage records for the whole state since June 1958 and sells certified copies at the counter, by mail, through a drop box at its building, or online through VitalChek, its only authorized online vendor.
A wedding from before June 1958 is not in the state file. For those, the cabinet tells people to call the clerk of the county that issued the license.4
How do you put a married name on a Kentucky driver license?
Only at a Driver Licensing Regional Office. The Transportation Cabinet rules out online and mail requests for a new name, and it will not process the application until the name matches Social Security records, so the SSA update comes first.
Bring the updated Social Security card and the marriage certificate. A pre-application on the myDrive portal is optional but can shorten the visit. You leave with a temporary document; the permanent card typically comes by mail in 10 to 15 business days.
If the new card will be a REAL ID, the certificate must be a certified copy, even if your current card already carries the married name. A marriage license does not count.1,2
What does Kentucky charge to change the name on a license?
The Transportation Cabinet lists a $15.00 fee for a name change, paid at the Regional Office where the change is made.1
How do you update a Kentucky voter registration with a new name?
The State Board of Elections says a new name has to be reported. Use govote.ky.gov, where a current Kentucky driver license helps fill in the form, or hand a paper Voter Registration Application to your County Clerk’s Office.
Timing matters: when the registration deadline for an election passes, the rolls are frozen and changes wait until that election is over.6,7

- Form SS-5 filled in from your answersYou print it, sign it and bring it to Social Security with your documents. The form itself is free on ssa.gov.
- The passport application for your case, filled inPicked from your answers about your current passport; the State Department’s fees are paid to the State Department. The application also needs a recent color photo, which the kit doesn’t include: U.S. passport photo rules, and a photo from a selfie.
- The Kentucky checklistThe steps above in order, with the deadline, the fee and the official page for each one.
- Notification lettersFor your bank, employer, insurer and other accounts, with your old and new names.
The kit does not file anything for you and is not legal advice. Government fees, like the license fee and the county copy of your marriage record, are paid directly to each office.

Not when the passport is less than a year old and the name changed within that year: Form DS-5504 has no fee unless you ask for expedited service or 1–2 day delivery.11
Can you bring a photocopy of your marriage certificate to Social Security?
No. Social Security accepts only originals or copies certified by the agency that issued them, and it refuses photocopies and notarized copies.3
