Questions about Ohio
Where do you get a certified copy of an Ohio marriage certificate?
From the probate court of the county where the marriage was recorded. The Ohio Department of Health’s Bureau of Vital Statistics keeps birth and death records but no marriage records, so it cannot send one.
Every county probate court keeps its own contact page online, which is where the ordering details live.6
How do you change your name on an Ohio driver license?
Buy a duplicate license at a deputy registrar license agency. Ohio’s online reprint only covers cards that need no changes, so a new surname always means a trip to an agency.
Bring the original or a certified copy of your marriage certificate or license to tie your birth certificate or passport to your married name; if you have married more than once, papers from each marriage or divorce may be needed. The application, Form BMV 5745, is signed in front of the agency employee.1,3,4
How much does a duplicate Ohio license cost?
The BMV fee table lists $29.00 for a duplicate operator license, deputy registrar fee included. Veterans with a 100 percent documented disability pay nothing.
A duplicate does not buy extra time: it carries the expiration date of the card it replaces. Drivers under 21 cannot request one in the 30 days before turning 21.1,2
How do you update an Ohio voter registration after a name change?
Ohio’s mail-in registration form also updates an existing record with a new name, and it must be postmarked 30 days before Election Day. Updates made in person, at an election office or the motor vehicles office, close on that same 30th day.
The Secretary of State’s online system lists only two jobs, new registrations and address updates; the mail-in form is the option that vote.gov ties to a new name. A change that misses the cutoff is not lost: it applies from the following election.8,9
What the Ohio kit does, and what it doesn’t
- Form SS-5 filled in from your answersYou print it, sign it and bring it to Social Security with your documents. The form itself is free on ssa.gov.
- The passport application for your case, filled inPicked from your answers about your current passport; the State Department’s fees are paid to the State Department. The application also needs a recent color photo, which the kit doesn’t include: U.S. passport photo rules, and a photo from a selfie.
- The Ohio checklistThe steps above in order, with the deadline, the fee and the official page for each one.
- Notification lettersFor your bank, employer, insurer and other accounts, with your old and new names.
The kit does not file anything for you and is not legal advice. Government fees, like the license fee and the county copy of your marriage record, are paid directly to each office.
Frequently asked questions
Can you order an Ohio marriage certificate from the state health department?
No. The Ohio Department of Health’s Bureau of Vital Statistics does not maintain marriage records. Ask the probate court in the county where the marriage was recorded.6
Can you change the name on an Ohio license online?
No. The BMV’s online reprint is only for cards that need no updates to details such as the name or address. A new name is handled as a duplicate license bought at a deputy registrar license agency.1
Does a duplicate Ohio license come with a new expiration date?
No. The duplicate keeps the expiration date printed on the card it replaces, so your renewal date does not move.1
What if you update your Ohio voter registration after the deadline?
The Ohio Secretary of State still records it, but the change applies from the next election rather than the one that closed 30 days out.8,9
Does it cost anything to change your name with Social Security?
No. Social Security mails the card with the new name at no charge. The paper application, Form SS-5, is published by Social Security itself.7,11
Is there a fee to change the name on a new passport?
Not when the passport is less than a year old and the name changed within that year: Form DS-5504 has no fee unless you ask for expedited service or 1–2 day delivery.13
Can you bring a photocopy of your marriage certificate to Social Security?
No. Social Security accepts only originals or copies certified by the agency that issued them, and it refuses photocopies and notarized copies.5
