Utah’s rules, with sources

Marriage certificate
  • Under the fee schedule in force since July 1, 2026, the Utah Office of Vital Records and Statistics charges $25.00 for the first certified copy of a marriage certificate.5
  • Extra copies of the same Utah record cost $10.00 each when they are ordered on the same day.5
  • The Utah state office only has marriages from 1978 to 2010; for a wedding before 1978 or after 2010, the county clerk where the marriage took place issues the copy.7
  • Utah’s 2026 request form adds that the state holds only limited marriage records from 2011 to the present, and that every fee paid is non-refundable.8
  • Most Utah local health departments take in-person orders for state vital records, with a complete application, the fee and valid ID.7
  • Utah Vital Records asks mail customers to allow at least six weeks for processing and mailing.7
Driver's license
  • The Utah Driver License Division lists $23.00 for a replacement regular Class D license, the card it issues for a lost license or an address or name change.1
  • The Utah Driver Handbook requires legal papers for any name that differs from the identity document, such as a state-certified marriage certificate, a divorce decree or an updated passport.2
  • The Utah Driver License Division takes only originals or certified copies with a raised seal, stamp or signature, and refuses photocopies and faxes.3
  • Utah warns that approving documents may take more than one day and a second visit to the Driver License Division.3
  • Utah driver license offices serve appointments first, so walk-in customers can face much longer waits.2
Voter registration
  • Utah’s voter registration portal lets a registered voter update an existing legal name, address or party affiliation.9
  • Updating a Utah voter record online takes a valid Utah driver license or state ID card.9
  • Utah also accepts the official paper form, which goes by mail or email straight to the county clerk’s office.9
  • After the Utah registration deadline, a voter can still register in person at an early voting location or on Election Day with two forms of ID.9

Social Security, taxes and passport

Social Security
  • The Social Security Administration recommends updating your Social Security record before you go to your state motor vehicle office.4
  • Social Security suggests waiting until the state has recorded the marriage, which takes about 30 days in most cases.4
  • For proof of identity, Social Security prefers a U.S. driver license and also accepts a state ID card or a U.S. passport.4
  • Social Security treats a marriage certificate as the ideal proof of both the new legal name and the marriage.4
  • Social Security accepts only original documents or copies certified by the issuing agency, never photocopies or notarized copies.4
  • Social Security does not charge for a card with the new name.6
  • Once Social Security completes the request, the replacement card arrives by mail in 5 to 10 business days.10
  • People who cannot use Social Security’s online services fill in the paper Form SS-5 and show the required documents.11
Passport
  • USAGov says to report a name change to the U.S. Department of State as soon as possible to get an updated passport.12
  • Form DS-5504 covers a name change by marriage when the passport was issued less than one year ago and the name changed within that year.13
  • The U.S. Department of State charges no fee for Form DS-5504 unless the applicant asks for expedited service or 1–2 day delivery.13
  • Form DS-82, the mail-in passport renewal, accepts a married name when the applicant sends certified proof such as a certified copy of the marriage certificate.14
  • Renewing with Form DS-82 requires sending the most recent passport, issued when the applicant was at least 16 and less than 15 years ago.14
  • An applicant who cannot document the name change with a certified marriage certificate, divorce decree or court order must apply on Form DS-11 instead.14
  • Form DS-11 is filed in person at a passport acceptance facility, at a passport agency by appointment, or at a U.S. embassy or consulate abroad.15
Voter registration
  • A voter registration can be updated online through vote.gov or by mail with the National Mail Voter Registration Form.12
Agencies
  • USAGov advises telling Social Security early because other agencies learn of name changes through the SSA.12
  • Every name on a federal tax return must match Social Security records.12

Questions about Utah

Where do you get a certified copy of a Utah marriage certificate?

It depends on the year. The Utah Office of Vital Records and Statistics only covers marriages from 1978 through 2010 (with a few later ones), so a couple married after 2010 orders from the clerk of the county where the wedding happened.

At the state office the first copy is $25.00 and each extra copy of the same record ordered that day is $10.00. Mail orders take at least six weeks, most local health departments accept walk-in requests, and fees are not refunded.5,7,8

What does a Utah driver license with your married name cost?

The Driver License Division bills it as a replacement: $23.00 for a regular Class D card, the same line that covers lost licenses and address changes.1

What does the Utah Driver License Division need to see?

A legal document linking your identity document to the new name; a state-certified marriage certificate is the usual one. It must be an original or a certified copy with a seal, stamp or signature: photocopies and faxes are refused.

Book an appointment, since walk-ins wait behind scheduled customers, and expect that a document review can stretch into a second visit.2,3

How do you change your name on a Utah voter registration?

Use the state’s online portal, which updates a legal name in a few minutes but asks for a valid Utah license or state ID card. Without one, send the paper form to your county clerk by mail or email, or fill it out at the clerk’s office.9

What the Utah kit does, and what it doesn’t

  • Form SS-5 filled in from your answersYou print it, sign it and bring it to Social Security with your documents. The form itself is free on ssa.gov.
  • The passport application for your case, filled inPicked from your answers about your current passport; the State Department’s fees are paid to the State Department. The application also needs a recent color photo, which the kit doesn’t include: U.S. passport photo rules, and a photo from a selfie.
  • The Utah checklistThe steps above in order, with the deadline, the fee and the official page for each one.
  • Notification lettersFor your bank, employer, insurer and other accounts, with your old and new names.

The kit does not file anything for you and is not legal advice. Government fees, like the license fee and the county copy of your marriage record, are paid directly to each office.

Frequently asked questions

Why can’t the Utah state office find my recent marriage?

The Utah Office of Vital Records and Statistics has marriages from 1978 to 2010 and only limited records after that. For a newer marriage, order from the county clerk where the wedding took place.7,8

Will the Utah Driver License Division take a photocopy of the marriage certificate?

No. It needs the original or a certified copy bearing a raised seal, stamp or signature; photocopies and faxed copies are listed as unacceptable.3

Do you need a Utah license to update your voter record online?

Yes, the online portal asks for a valid Utah driver license or state ID card. Without one, use the paper form and send it to your county clerk.9

Does it cost anything to change your name with Social Security?

No. Social Security mails the card with the new name at no charge. The paper application, Form SS-5, is published by Social Security itself.6,11

Is there a fee to change the name on a new passport?

Not when the passport is less than a year old and the name changed within that year: Form DS-5504 has no fee unless you ask for expedited service or 1–2 day delivery.13

Can you bring a photocopy of your marriage certificate to Social Security?

No. Social Security accepts only originals or copies certified by the agency that issued them, and it refuses photocopies and notarized copies.4
