Kit $34.95 per state · 51 states published
Name change after marriage, state by state
As of November 3, 2026, a name change after marriage starts with a certified copy of the marriage record and then Social Security, which asks newlyweds to update their record before the motor vehicle office.1
The driver license, the voter registration and the marriage record follow state rules, so the deadline and the fee change from one state to the next.

The order matters
Social Security is the hinge: other agencies check your name against it, so everything after it goes faster.
County clerk or state vital records
Certified copy of the marriage record
Order it first. Social Security accepts only originals or copies certified by the issuing office, never photocopies.1
Your state motor vehicle office
Driver’s license or state ID
Each state sets its own deadline, fee and documents. Your state page has them, with the source for each.
Your state election office
Voter registration
Update it online through vote.gov or by mail with the National Mail Voter Registration Form.2
Payroll, accounts, insurance
Employer, bank and everyone else
Tell your employer for payroll: every name on a tax return must match Social Security records.2

What’s in the kit
One state, $34.95, paid once. The forms are filled in from your answers; you sign them and hand them in yourself.
Government forms are free on each agency’s website. You pay for the preparation, never for the form.
SimplePaperwork is not a law firm and does not give legal advice. We explain public forms and official steps, and you decide what to sign and file.

Federal rules
Social Security, taxes and passport
- Social Security
- The Social Security Administration recommends updating your Social Security record before you go to your state motor vehicle office.1
- Social Security suggests waiting until the state has recorded the marriage, which takes about 30 days in most cases.1
- For proof of identity, Social Security prefers a U.S. driver license and also accepts a state ID card or a U.S. passport.1
- Social Security treats a marriage certificate as the ideal proof of both the new legal name and the marriage.1
- Social Security accepts only original documents or copies certified by the issuing agency, never photocopies or notarized copies.1
- Social Security does not charge for a card with the new name.3
- Once Social Security completes the request, the replacement card arrives by mail in 5 to 10 business days.101
- People who cannot use Social Security’s online services fill in the paper Form SS-5 and show the required documents.102
- Passport
- USAGov says to report a name change to the U.S. Department of State as soon as possible to get an updated passport.2
- Form DS-5504 covers a name change by marriage when the passport was issued less than one year ago and the name changed within that year.4
- The U.S. Department of State charges no fee for Form DS-5504 unless the applicant asks for expedited service or 1–2 day delivery.4
- Form DS-82, the mail-in passport renewal, accepts a married name when the applicant sends certified proof such as a certified copy of the marriage certificate.5
- Renewing with Form DS-82 requires sending the most recent passport, issued when the applicant was at least 16 and less than 15 years ago.5
- An applicant who cannot document the name change with a certified marriage certificate, divorce decree or court order must apply on Form DS-11 instead.5
- Form DS-11 is filed in person at a passport acceptance facility, at a passport agency by appointment, or at a U.S. embassy or consulate abroad.103
- Voter registration
- A voter registration can be updated online through vote.gov or by mail with the National Mail Voter Registration Form.2
Frequently asked questions
Does it cost anything to change your name with Social Security?
No. Social Security mails the card with the new name at no charge. The paper application, Form SS-5, is published by Social Security itself.3,102
Is there a fee to change the name on a new passport?
Not when the passport is less than a year old and the name changed within that year: Form DS-5504 has no fee unless you ask for expedited service or 1–2 day delivery.4
Can you bring a photocopy of your marriage certificate to Social Security?
No. Social Security accepts only originals or copies certified by the agency that issued them, and it refuses photocopies and notarized copies.1
