Questions about Illinois
Where do you get a certified copy of an Illinois marriage certificate?
From the county clerk in the county where the wedding took place. The Illinois Department of Public Health keeps no marriage records, so the state office in Springfield cannot send a certified copy.
What IDPH sells is a $5 verification of the facts of a marriage recorded since 1962, by mail, fax or in person, with a readable photo ID. It lists names, birth dates and the date and place of the event; the certified copy itself still comes only from the county clerk.5
How long do you have to report a new name to the Illinois Secretary of State?
Ten days. The Secretary of State writes that the law requires notice of a name change within 10 days, either at a DMV facility or by letter to the Driver Services Department in Springfield. Commercial drivers must also get a corrected CDL within 30 days.
The online change form is for addresses only; a new name on a license or ID card has to be finished at a DMV.1,2
How much does a corrected Illinois license or ID cost?
Five dollars for the corrected driver’s license, paid at the driver services facility. The Secretary of State’s fee table drops it to $2 for drivers aged 81 to 86 and charges nothing from age 87.
A corrected state ID card is $10, or $5 under age 18, and free for anyone 65 or older.2,3
What happens at an Illinois DMV when you change your name?
You turn in the old license, show ID in your new name plus a document tying the two names together, and pay $5 for the corrected license. Before anything is printed, the office checks your name, birth date and Social Security number with the Social Security Administration and stops if they do not match.
You walk out with a temporary license on secure paper, valid for 90 days; the permanent card follows by mail within 15 business days. For flights in the meantime, the DMV gives the old card back, and Homeland Security accepts it together with the paper license.1,2
Do you have to re-register to vote in Illinois after a name change?
Yes, and the calendar matters. According to the State Board of Elections, a voter whose name changed more than 27 days before an election and who did not re-register cannot vote in it, unless still living in the same precinct.
Regular registration closes 27 days before an election, online registration 16 days before, and Grace Period registration covers the stretch after that. The license application at a Secretary of State facility also offers to update your voter record.7–9
What the Illinois kit does, and what it doesn’t
- Form SS-5 filled in from your answersYou print it, sign it and bring it to Social Security with your documents. The form itself is free on ssa.gov.
- The passport application for your case, filled inPicked from your answers about your current passport; the State Department’s fees are paid to the State Department. The application also needs a recent color photo, which the kit doesn’t include: U.S. passport photo rules, and a photo from a selfie.
- The Illinois checklistThe steps above in order, with the deadline, the fee and the official page for each one.
- Notification lettersFor your bank, employer, insurer and other accounts, with your old and new names.
